This is an example of a Chaplet, a shorter version of a Rosary. Chaplets are usually dedicated to a particular Saint of choice. 

A 'Niner' is made up of three sets of three. On each set of three beads, one each you say: Our father, Hail Mary, Glory Be. 

On the Medal: Ask for the intercession of this particular Saint. 

Some people use this as a Novena by adding their petition at this time.

This Chaplet is made up of carnelian, bone and seed beads and hand knotted on silk thread. The John Neumann medal is sterling silver.
